This project builds a tool for primary care physicians to know where to find information for adolescent care when they need it. Patients experience a significant transition within primary health care when they turn 18, transitioning from children’s to adult primary care. This transition often has bad results for patients, especially for children with long-term conditions. This project will search previous studies and get advice from physicians, and observe physicians using to tool. This will enable our team to apply to do a bigger project where we apply and improve the tool to better serve young people in primary care.
