Improving primary health care access for vulnerable populations: Spread of a peer-support navigational intervention

Résumé

This project adapts in one clinic and spreads to a new clinic a promising intervention in which lay volunteers provide navigational support by telephone to help patients access needed health services. The initial test of the intervention shows that the telephone support was highly appreciated by patients and that it improved their capacity to access health services. The volunteer guides found the experience both meaningful and feasible. We will verify the intervention’s effectiveness, feasibility and acceptability in a new set of patients, volunteers, and clinicians by a process of ongoing evaluation that adapts the intervention and embeds it in the clinics’ existing routines.
